LSWA offers help with getting a Maryland Birth Certificate for employment. This program is exclusively designed for residents of Maryland who were born within the state and require a Maryland Birth Certificate. Contact us to schedule an appointment.
ID Requirements
You must have a valid, unexpired. government-issued photo ID. If you cannot present a valid photo ID you MUST provide (2) distinct pieces of alternative documentation. It is important that at least one of these documents should have your current mailing address on it. Acceptable documents for identification include:
- Pay stub
- Current car registration
- Bank statement
- Letter from a government agency requesting a vital record
- Lease/rental agreement
- Utility bill with current address
- Copy of income tax return/W-2 form
- Social Security card
- Student ID
